I hereby authorize the repairs an/or services shown to be performed on the vehicle identified above, which may include the installation of parts and materials and/or tear-down. I further authorize you to perform diagnostics and testing to ascertain the cause and correction of my complaint, when necessary for which a charge may be incurred. I understand you will provide me with an estimate of charge for my approval for any work not specified on this check-in form. I further understand that should I decline recommended repairs and services, diagnostic/tear-down charges incurred may be due. I agree to these charges without dispute. I hereby authorize you and/or your employee’s permission to operate and drive my vehicle to the extent you deem necessary to inspect and/or test the vehicle. I acknowledge and agree that APEX AUTO COLLISION shall not be responsible or liable for any loss of the vehicle or any articles left in the vehicle. There will be a 15% hazmat fee.
